Before scheduling we will need your latest x-rays from your old dentist with the dates they were taken. Please call your old dentist and have the office email your x-rays to: xraysforpatients@gmail.com

Periodically, call our office to make sure we have received the x-ray. As soon as we receive the x-rays make sure you fill out all the new patient paperwork, three forms in all, because if we do not receive all the forms we cannot schedule your appointment. Once all your forms are filled out you can give our office a call to schedule your first appointment. Also, print, sign, and bring to the office with you the Office Policy and Consent Form and the HIPPA Privacy Policy Form.
