Our office is doing everything possible to make your dental experience as pleasant as possible. We are happy to introduce the ease of requesting your appointments through the website and for you to make sure all the personal and medical information we have on file is always up-to-date and correct. 

If you have not done so already, please create your portal account. Within the portal you can request an appointment and you will be able to update all your forms 24 hours after you have a scheduled appointment. Unfortunately, with Dentrix, if you do not have an appointment scheduled you will not be able to see the forms. Also, read carefully for our office policy has changed, then print, sign, and bring to the office with you the Office Policy and Consent From and the HIPPA Privacy Policy Form.
